The undersigned hereby makes application for
membership in and agrees to pay upon acceptance to the Warrenton
Area Chamber of Commerce annual dues as follows:

These dues are for the first representative
(voting member of chamber) of your company. Additional representatives
dues are $100.00 each regardless of your first representative's
dues level.
